Health Canada has posted a medical device safety alert concerning Vial Adapter for the Advantajet, Gentlejet, and Advantajet ES manufactured by Activa Brand Products Inc.. The affected products can be identified by the model/catalogue number 13000 and lot no.: AB10 respectively.
According to the report, a recall of the devices has been initiated due to the product being unsterile. Use of the product may cause infection.
